Responsibilities Assist individuals and families by connecting them with appropriate healthcare resources and community services Coordinate and facilitate community outreach programs, workshops, and health education sessions Conduct assessments to identify client needs, develop personalized care plans, and monitor progress Collaborate with healthcare providers, social workers, and local organizations to ensure seamless service delivery Maintain accurate documentation of client interactions, service plans, and program activities Support event planning and logistics for community health initiatives to maximize participation and impact Provide empathetic support and guidance to clients navigating complex health or social challenges Experience Previous experience in healthcare, social services, or community outreach is highly preferred Strong interpersonal skills with the ability to communicate effectively across diverse populations Knowledge of local healthcare systems, social services networks, and community resources Ability to assess client needs accurately and develop tailored support strategies Excellent organizational skills with attention to detail for record keeping and program coordination Demonstrated commitment to promoting equity, inclusion, and cultural sensitivity in service delivery This paid position offers a rewarding environment where your dedication can truly transform lives.
