Responsibilities Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and mental health needs, including diagnostic evaluation and behavioral health screening. Develop personalized care plans incorporating clinical counseling, group therapy, and motivational interviewing techniques to support patients with conditions such as dementia, Alzheimer's, PTSD, or developmental disabilities. Provide individual and family counseling sessions focusing on coping strategies, addiction counseling, crisis intervention, and behavioral therapy tailored to each patient's unique circumstances. Collaborate with interdisciplinary teams to coordinate home and community-based care, ensuring compliance with HIPAA regulations and accurate medical documentation in electronic medical records (EMR) systems. Assist with intake procedures for new patients, gather data for clinical research projects, and contribute to quality improvement initiatives related to hospice care and post-acute services. Support patients through complex transitions such as assisted living placements or inpatient discharges by offering case management and social work expertise. coding for diagnoses, CPT coding for services rendered, and ensure all documentation aligns with healthcare standards. Requirements Bachelor's or Master's degree in Social Work (MSW preferred) from an accredited program; licensure or certification as required by state law. Proven experience in home health or hospice settings with familiarity in geriatrics, palliative care, dementia care, or pediatrics. Strong knowledge of medical terminology, ICD coding systems (ICD-9/10), CPT coding, and HIPAA compliance standards. Demonstrated skills in child & family counseling, crisis management, behavioral health interventions, and working with individuals with disabilities or developmental delays. Experience utilizing EMR systems for documentation and data collection; proficiency in managing medical records securely. Excellent communication skills for engaging with patients of diverse backgrounds—including children—and collaborating effectively within multidisciplinary teams. Ability to handle sensitive situations with empathy while maintaining professional boundaries; experience in inpatient or outpatient clinical settings is highly valued.
