This vital role involves providing attentive, respectful, and professional support to promote independence, safety, and well-being. As a Home Health Aide, you will be an essential part of a dedicated healthcare team, delivering high-quality care that enhances quality of life for your clients. This position offers an engaging opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those recovering from illness or managing chronic conditions. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure comfort and dignity Monitor patient observations including vital signs, mental status, and overall health status to detect changes or concerns early Support medication administration following prescribed instructions while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems accurately for medical documentation and record keeping Provide specialized care for clients with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Utilize assistive devices such as Hoyer lifts and catheter care tools safely during patient transfers and hygiene routines Perform light housekeeping tasks like cleaning, laundry, and meal preparation to maintain a safe and healthy environment in accordance with home & community care standards Document all patient interactions thoroughly in medical records and ensure compliance with HIPAA guidelines for data security Skills Proven experience in home health or long-term care environments with a focus on senior care or hospice services Strong knowledge of medical terminology, vital signs measurement, medication administration, and dementia or Alzheimer's care techniques Skilled in working with individuals with developmental disabilities or other special needs; experience in assisted living or nursing home settings is highly valued Familiarity with EMR/EHR systems for accurate medical documentation; ability to navigate electronic records efficiently CPR certification and training in hospice or palliative care practices to provide compassionate end-of-life support when needed Ability to operate assistive devices such as Hoyer lifts safely; experience with catheter care is a plus Excellent patient observation skills to identify changes in condition promptly and communicate effectively with healthcare teams Capable of working independently while maintaining a caring attitude; flexibility for live-in employment arrangements is advantageous Join us if you're passionate about caregiving and eager to provide exceptional support that improves lives every day.
