Job Description
Job Summary Join our dedicated healthcare team as a Home Health Aide & Caregiver, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals needing support at home. In this vital role, you will provide personalized assistance with daily activities, monitor patient health, and ensure a safe, comfortable environment. Your energetic approach and commitment to quality care will help clients maintain their independence and improve their well-being. This paid position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care. Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor and record vital signs, medication administration, and changes in patient condition using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Support patients in assisted living or long-term care settings by helping with meal preparation, light cleaning, laundry, and maintaining a hygienic environment Observe patient conditions closely, noting any changes or concerns related to medical terminology or symptoms for accurate medical documentation Assist with catheter care, hospice support, and other complex medical needs following proper protocols and HIPAA regulations Use equipment such as Hoyer lifts and perform safety checks to ensure patient mobility and prevent falls or injuries Communicate effectively with healthcare teams regarding patient status and document all care activities thoroughly in medical records Qualifications Prior experience in home health care, nursing homes, assisted living facilities, or long-term care environments is highly preferred Knowledge of medical terminology, vital signs monitoring, medication administration, and HIPAA compliance Certifications such as CPR (Cardiopulmonary Resuscitation), first aid training, and specialized caregiving certifications are required or highly valued Experience working with individuals with developmental disabilities or disabilities is advantageous Ability to perform patient observation accurately and assist with complex medical procedures like catheter care or hospice support Strong communication skills to collaborate effectively with patients, families, and healthcare professionals Demonstrated compassion, patience, and professionalism in providing high-quality patient care Join us in delivering exceptional home health services that empower clients to live safely and comfortably at home.
