Job Description

Overview Join our dedicated team as a Home Health Aide & Caregiver, where your compassion and skills will make a meaningful difference in the lives of individuals requiring personalized care. In this vital role, you will provide essential support to clients in their homes or community settings, ensuring their safety, comfort, and well-being. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those managing chronic conditions such as Alzheimer's disease or dementia. Your efforts will promote independence and enhance quality of life through attentive, respectful care. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Monitor and document patient observations, vital signs, and medical conditions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications and perform specialized care procedures such as catheter care, Hoyer lift transfers, and hospice support under supervision or as trained. Support clients with memory care needs by implementing personalized routines for Alzheimer's and dementia patients to ensure safety and comfort. Maintain detailed medical documentation in compliance with HIPAA regulations to ensure confidentiality and accuracy of patient records. Prepare nutritious meals aligned with dietary restrictions and assist with meal planning and preparation to promote health. Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to support overall well-being. Collaborate with healthcare teams to communicate changes in patient conditions and update medical records promptly. Use medical terminology accurately when recording observations or communicating with healthcare professionals. Employ proper infection control practices during patient care activities to uphold safety standards. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to work effectively with individuals who have developmental disabilities or require specialized dementia or Alzheimer's care. Familiarity with medical procedures such as vital signs measurement, medication administration, catheter care, and hospice support. Experience utilizing EMR/EHR systems for documentation and patient record management. Strong understanding of patient observation techniques and the ability to recognize changes in health status promptly. Knowledge of medical terminology relevant to home health caregiving roles. Certifications such as CPR (Cardiopulmonary Resuscitation) are required; additional certifications like Hoyer lift training or hospice care experience are advantageous. Long-term care experience is beneficial for providing consistent, high-quality support over extended periods.
