Important Requirement:
All new hires must complete 7 days of orientation (8:30 AM - 4:00 PM) prior to starting at a residential or day program site.Position:

Monday-Friday 9:00am-2:30pm Position Summary The Habilitative Support Professional (HSP) provides person-centered support to individuals with developmental disabilities in adult day services, helping them achieve goals, build skills, engage in the community, and maintain their health, safety, and independence. Essential Responsibilities & Duties - Professionalism & Conduct Maintain professional behavior and follow agency mission, policies, and procedures Treat individuals and coworkers with dignity and respect at all times Support Individuals to Achieve Personal Outcomes and a High Quality of Life Conduct regular assessments of individuals' interests, goals, and abilities; document findings for Team and Annual Plan meetings Support skill development through formal and informal training in alignment with the individual's person-centered plan; accurately document progress and report changes to the ADS Coordinator Assist with activities of daily living, including toileting and personal hygiene, in a manner that upholds individuals' dignity, choice, and privacy. Encourage informed decision-making and promote participation in community-based activities Promote an environment that is free from abuse and neglect, respecting the rights and dignity of individuals and coworkers Health & Safety Support Provide direct supervision of individuals, ensuring accountability and documentation as required by the person-centered plan and agency policy. Use positive behavior support techniques and implement Behavior Support Plans (as applicable) to manage and de-escalate challenging situations Monitor and report changes in individuals' health and daily routines Support prescribed diets, adaptive equipment use, and safety drills Follow proper lifting and mobility assistance techniques Documentation & Communication Complete accurate documentation in Therap and other required forms (e.g., T-Logs, GERs, ISP data) Communicate changes or incidents to supervisors and nursing staff Maintain confidentiality and protect agency-issued credentials Transportation & Community Engagement Safely transport individuals to activities, and outings Operate agency vehicles, wheelchair lifts, and restraints as trained Follow agency transportation and safety protocols Medication Administer medications and treatments per agency policy Complete Medication Administration Records (MARs) Additional Duties Report maintenance or repair needs Attend meetings and complete ongoing training as required Other duties as assigned Work Activity Program Duties Accurately complete consumer time sheets, and wage assessments in accordance with U.S. Department of Labor Standards Monitor work quality to meet or exceed the customers specifications and participate in work completion as needed Provide training and support to individuals to maximize vocational capabilities and complete Time Studies as needed Submit shipping and receiving documentation to the Administrative Assistant in a timely manner Support Alliant licensing reviews by ensuring all required documentation is complete, accurate, and readily available Lead HSP Duties Ensure all Habilitative Support Professional documentation is accurate, complete, and correct per agency standards. Support Alliant licensing reviews by ensuring all required documentation is complete, accurate, and readily available Assist in the review and implementation of contract-related activities and individual support programs Physical Demands and Working Conditions Lift up to 50 lbs; assist with adult transfers as needed Frequent movement: standing, walking, bending, kneeling, crouching, reaching, and lifting Work indoors and outdoors in varied environments Possible exposure to communicable diseases and physical aggression Use of safety equipment (gloves, eye/ear protection) may be required May be assigned to alternate locations or shifts Must operate agency vehicles safely, including at night Performance Requirements - Knowledge, Skills, & Abilities Clear written and verbal English communication Ability to follow directions and work independently or/ as a team Positive interpersonal skills and time management Good judgment and accountability Education & Experience High School Diploma or GED (required) and Minimum age: 18 One year working with individuals with developmental disabilities (preferred) License Valid South Carolina Driver's License and acceptable driving record required Essential Personnel Notice This position is classified as Essential.
