Job Description

Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care for clients with diverse needs. In this vital role, you will support individuals in maintaining their independence and quality of life by assisting with daily activities, monitoring health status, and ensuring a safe and comfortable environment. Your energetic approach and attention to detail will make a meaningful difference in the lives of those you serve. This position offers an opportunity to work closely with clients, families, and healthcare teams to deliver personalized care that promotes well-being and dignity. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary. Monitor patient observations including vital signs, mental status, and overall health condition; document findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications following prescribed schedules while adhering to HIPAA regulations and safety protocols. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral management. Support clients with medical procedures such as catheter care, wound dressing changes, and hospice care as directed by healthcare professionals. Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking; ensure cleanliness of the home environment through light cleaning and laundry duties. Maintain detailed medical documentation, update patient records regularly, and communicate effectively with healthcare teams regarding patient progress or concerns. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to work with individuals with developmental disabilities or disabilities requiring long-term care. Knowledge of medical terminology, medication administration procedures, and vital signs monitoring is essential. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or dementia certification are a plus. Experience operating EMR/EHR systems for accurate documentation; familiarity with HIPAA compliance standards is necessary. Proven ability to handle physically demanding tasks such as lifting patients using Hoyer lifts or other mobility aids safely.
