In this vital role, you will foster independence, promote well-being, and ensure a safe and nurturing environment. Duties Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or resident's personal assistance devices Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately while adhering to HIPAA privacy regulations Prepare nutritious meals and assist with meal planning in accordance with care plans and dietary restrictions Support residents in engaging with community activities, social outings, and recreational programs to promote socialization and mental stimulation Administer medications following prescribed protocols and document all medication administration accurately Maintain a clean, safe environment by performing light housekeeping duties such as cleaning resident areas, laundry, and ensuring infection control standards are met Follow care plans meticulously, documenting progress and any changes in condition while collaborating with healthcare teams including nurses and social workers Use proper techniques for patient observation and monitoring to ensure safety at all times, especially when working with individuals experiencing memory loss or cognitive decline such as Alzheimer's or dementia Experience Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care settings is highly preferred Knowledge of patient care procedures including CPR and first aid certification is essential for ensuring safety during emergencies Demonstrated experience working with individuals with developmental disabilities or autism spectrum disorder (ASD) is advantageous Familiarity with medication administration protocols and HIPAA compliance is required for maintaining confidentiality and safety standards Experience providing dementia or memory care services enhances your ability to meet complex needs effectively Ability to perform heavy lifting safely using equipment like Hoyer lifts when assisting residents with mobility challenges Strong communication skills developed through social work or caregiving roles that involve collaborating with families, healthcare providers, and team members Join us as a Direct Support Professional where your dedication transforms lives!
