Personal Healthcare Assistant

Regents Medical Center is looking for experienced clinical staff to join our team as a Personal Healthcare Assistant . As the Personal Healthcare Assistant, you will be responsible for engaging with patients monthly by telephone to review their care plan and chronic conditions. You will work as an extension of their physician to provide exceptional care in between their scheduled office visits. Must pass drug screen and back ground check.

Responsibilities:

Performing monthly calls to over 300 Medicare patients. The monthly job duties will include but not limited to: Updating patient care plans Preparing patients and providers for upcoming visits Chart review Medication reconciliation Referral coordination Patient education Prescription refill requests Meeting with patients in office if they do not respond over the phone Other activities designed to close gaps in care( HEDIS

Requirements:

Experience in a clinical setting preferred Must have exceptional communication skills, while maintaining professionalism and etiquette Must be self-motivated, reliable and hardworking- showing initiative to complete tasks when needed Must work well in a team environment Must be adaptable and thrive in a fast-paced environment

$17.00 - $18.00 per hour
