Job Description
Overview Join our compassionate team as a Home Health Aide and make a meaningful difference in the lives of individuals needing personalized care at home. In this vital role, you will provide hands-on assistance, monitor health status, and support independence for clients across diverse care settings. Your dedication will help promote comfort, safety, and well-being while fostering trusting relationships with those you serve. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices. Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately. Administer medications following prescribed schedules and document all medical records in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining HIPAA compliance. Support clients with specialized needs such as dementia care, Alzheimer's care, hospice care, or working with individuals with developmental disabilities and disabilities. Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment conducive to health and comfort. Utilize medical terminology appropriately when documenting patient care and communicating with healthcare teams. Collaborate with nurses and other healthcare professionals to ensure comprehensive patient care plans are followed effectively. Skills Proven experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience. Strong knowledge of medical procedures including catheter care, medication administration, and hospice support. Familiarity with EMR/EHR systems for accurate documentation of patient information and medical records. Excellent patient observation skills to identify changes in condition or behavior promptly. Ability to work independently or as part of a team providing home & community care services. Compassionate caregiving skills tailored to seniors, individuals with disabilities, or those requiring memory or dementia care. Knowledge of medical terminology and CPR certification to ensure safety during emergencies.
