As a Hospice Aide, you will play a vital role in enhancing the quality of life for patients by delivering personalized support, assisting with daily activities, and ensuring comfort and dignity. This position offers an opportunity to make a meaningful difference in the lives of patients and their families through empathetic caregiving and attentive medical support. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance using tools such as Hoyer lifts and other adaptive equipment. Monitor patient condition by observing vital signs, changes in health status, and overall well-being, documenting findings accurately in Electronic Health Record (EHR) systems. Support patients with medication reminders, basic medication administration under supervision, and catheter care as needed, ensuring adherence to medical protocols. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques and compassionate engagement. Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment that promotes patient comfort and safety. Maintain strict compliance with HIPAA regulations to protect patient privacy and confidentiality at all times. Collaborate closely with nursing staff and other healthcare professionals to ensure comprehensive patient care plans are followed effectively. Qualifications Prior experience in nursing homes, assisted living facilities, home health care, or hospice care is highly desirable. Knowledge of medical terminology, vital signs measurement, medication administration, and medical documentation is essential. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice or dementia care are a plus. Experience working with individuals with disabilities or developmental challenges enhances your ability to deliver personalized care. Strong observational skills for patient monitoring and the ability to respond swiftly to changing conditions are crucial. Ability to work independently while maintaining excellent communication with healthcare teams. Valid driver's license and reliable transportation preferred for home visits; willingness to work flexible hours including live-in shifts if necessary.
