This position operates on an as-needed basis. Work hours are not guaranteed and will vary depending on business needs, staffing requirements, and client demand. Position Summary The PRN Transportation Staff Team Member provides safe and reliable transportation services for individuals participating in the Home and Community-Based Services (HCS) program. This role may also support direct care and daily living activities as needed. All services must be delivered in accordance with Medicaid Waiver requirements and applicable regulatory standards. Essential Duties and Responsibilities Safely transport individuals to and from appointments, activities, and program locations. Perform basic office duties such as copying, filing, and organizing documents. Assist individuals with activities of daily living, including grooming, dressing, eating, hygiene, and mobility as needed. Support meal preparation, housekeeping, and general care needs. Assist with ambulation and reinforce therapy or counseling-related activities. Support medication administration and RN-delegated tasks as assigned. Ensure the health, safety, and well-being of all individuals served. Promote community inclusion, social interaction, and use of natural supports. Maintain professional appearance and adhere to attendance and timekeeping policies. Report incidents, injuries, and concerns in a timely and accurate manner. Maintain confidentiality of all client and organizational information. Perform additional duties as assigned by management. Individual Plan Development and Implementation Participate in the development and implementation of Personal Development Plans (PDPs). Communicate relevant updates, concerns, and observations to the Interdisciplinary Team (IDT). Complete documentation accurately and timely, including progress notes and incident reports. Follow all safety policies to ensure a secure and supportive environment free from abuse or harm. Core Competencies Teamwork:

Valid Texas driver's license with acceptable driving record. Ability to pass background check. Ability to read, write, and communicate effectively in English. Demonstrated ability to provide a safe and supportive environment. Physical Requirements Ability to lift up to 75 pounds. Frequent standing, walking, bending, and reaching. Ability to safely operate vehicles and office equipment. Work may occur in residential settings and vehicles with varying environmental conditions.
