Job Description

Job Overview Join our dedicated home health team as a compassionate and proactive Home Health Caregiver! In this vital role, you will provide personalized, high-quality care to individuals in their homes, supporting their health, safety, and independence. Your energy and attention to detail will make a meaningful difference in the lives of patients with diverse needs, including those requiring memory care, hospice support, or assistance with daily activities. This position offers an opportunity to work closely with clients and their families, fostering a safe and comfortable environment while promoting well-being and dignity. Responsibilities Deliver comprehensive patient observation to monitor health status and identify any changes or concerns promptly. Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support. Manage medication administration accurately according to prescribed medical plans while maintaining detailed medical documentation. Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to record patient information securely and efficiently. Provide specialized care for individuals with dementia, Alzheimer's disease, or other cognitive impairments, including memory care techniques. Perform catheter care, vital signs monitoring, and assist with medical procedures like Hoyer lift transfers or hospice-related tasks. Support clients with meal preparation, cooking, laundry, light cleaning, and maintaining a safe home environment in compliance with HIPAA regulations. Utilize medical terminology confidently when documenting patient records and communicating with healthcare teams. Follow all safety protocols including CPR certification requirements and infection control standards to ensure patient safety at all times. Work effectively within assisted living facilities or long-term care environments when necessary. Experience Prior experience in home health caregiving or related settings such as nursing homes or assisted living communities. Demonstrated ability to work with individuals with developmental disabilities or those requiring specialized dementia and Alzheimer's care. Knowledge of medical procedures including medication management, vital signs assessment, catheter care, and hospice support. Familiarity with EMR/EHR systems for accurate documentation of patient information. Strong understanding of medical terminology relevant to home health services. Experience working with patients needing long-term care or live-in employment arrangements is preferred. Ability to handle physically demanding tasks such as lifting patients using Hoyer lifts and assisting with mobility needs safely.
