Job Description

Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide high-quality care to individuals in their homes and community settings. The ideal candidate will possess a strong commitment to patient well-being, with experience in personal care, medical assistance, and supporting individuals with diverse needs. As a Home Health Aide, you will play a vital role in enhancing the quality of life for clients by assisting with daily activities, monitoring health status, and ensuring a safe and comfortable environment. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting. Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates accurately using Electronic Health Record (EHR) systems. Support patients with medication administration following prescribed guidelines while maintaining compliance with HIPAA regulations. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored activities and attentive supervision. Perform light housekeeping tasks including cleaning, laundry, and meal preparation to promote a safe and hygienic living environment. Assist with medical procedures such as catheter care, Hoyer lift transfers, and hospice support as needed under supervision. Document patient progress and medical records meticulously to ensure continuity of care and compliance with healthcare standards. Collaborate effectively with nursing staff and other healthcare professionals to deliver comprehensive patient care in home or assisted living settings. Qualifications Proven experience as a Home Health Aide or in long-term care environments such as nursing homes or assisted living facilities. Strong knowledge of medical terminology, patient observation techniques, and caregiving best practices. Certification in CPR is required; additional certifications such as medication administration or hospice care are advantageous. Experience working with individuals with developmental disabilities or disabilities is preferred. Familiarity with EMR (Electronic Medical Records) systems and home & community care practices. Ability to perform physical tasks including lifting using Hoyer lifts and assisting with mobility needs safely. Excellent communication skills coupled with compassionate patient care approach.
