Caregiver/ Home health Aide Revora Health Systems Rockwall, TX Job Details Temporary | Part-time | Full-time | Contract $16 - $18 an hour 12 hours ago Benefits Mileage reimbursement Flexible schedule Qualifications Working with individuals with developmental disabilities Spanish CPR Certification Patient transfer methods High school diploma or GED Driver's License Patient interaction CNA First aid and CPR Full Job Description Job Summary The Caregiver / Home Health Aide provides personal assistance and non-medical support services to clients in their homes in accordance with each client's individualized Plan of Care, Revora's agency policies, and Texas HHSC regulations under TAC Chapter 558. This role requires professionalism, reliability, and genuine compassion; you are often the most consistent point of contact a client has each week. Responsibilities Personal Care & Daily Living Assistance Assist clients with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and mobility as specified in the Plan of Care. Plan of Care Compliance Deliver all services strictly in accordance with the client's documented Plan of Care, which includes medical diagnoses, functional limitations, permitted activities, nutritional requirements, medications, treatments, and safety measures. Do not perform any task not authorized in the client's Plan of Care or delegated in writing by the supervising RN. Report any change in client condition, behavior, or health status to the supervising RN promptly. Client Rights & Dignity Respect each client's right to refuse any service or treatment at any time — document refusals clearly and notify the supervising RN. Maintain client privacy, confidentiality, and dignity in all interactions. Recognize and honor the authority of a client's Medical Power of Attorney (MPOA) when applicable. Do not make physical, verbal, or emotional contact that could be construed as abuse, neglect, or exploitation. Documentation & Reporting Complete accurate, timely service logs for every visit — documenting tasks performed, client status, and any observations relevant to the client's care. Report any suspected or confirmed abuse, neglect, or exploitation of a client to the supervising RN and/or Texas APS within 24 hours — this is a mandatory legal obligation under Texas law. Complete incident reports for any client injury, fall, or unexpected change in condition using Revora's incident report form. Provide written and verbal sharps disposal instructions to any client who uses sharps (needles, lancets, syringes) and document in the client record. Emergency Preparedness Know and follow Revora's written emergency preparedness plan for fire, natural disasters, power outages, and medical emergencies. In the event of a medical emergency, follow the documented response procedure and ensure the client receives appropriate medical attention within the required timeframe. For a dislodged
G-tube:
follow the documented emergency protocol and ensure the client receives medical attention within 1 hour. Know who to contact at Revora for each emergency type and keep emergency contact information accessible during every shift. Qualifications Prepare meals and assist with feeding per client dietary requirements and physician orders. Perform light housekeeping duties to maintain a safe, clean client environment. Assist with ambulation, transfers, and repositioning using safe body mechanics and approved techniques. Prior experience in home health care, assisted living, nursing homes, or group homes is highly preferred Knowledge of patient observation, vital signs measurement, medication administration, and HIPAA compliance Certifications such as CPR and First Aid are required; additional training in Alzheimer's or dementia care is a plus Ability to lift heavy objects safely and perform physically demanding tasks regularly Experience working with individuals with developmental disabilities, autism, or other special needs is advantageous Strong communication skills to effectively interact with clients, families, and healthcare teams Compassionate attitude with a commitment to providing respectful, person-centered care Valid driver's license and reliable transportation are often necessary for community-based roles Required High school diploma or GED. Completion of a minimum 75-hour home health aide training program (at least 16 hours of clinical/hands-on training) — or ability to complete training before first client assignment, as required by Texas TAC Chapter 558. Ability to pass a Texas DPS criminal history check and HHSC Employee Misconduct Registry (EMR) check with no disqualifying findings. Valid Texas driver's license and reliable personal transportation. Ability to physically assist clients with transfers, ambulation, and personal care — including lifting and standing for extended periods. Strong communication skills — ability to follow written care instructions and report observations clearly. Preferred Certified Nurse Aide (CNA) certification — Texas Nurse Aide Registry in good standing (24 hours of in-service training every 2 years required to maintain active status). Prior home care, personal care, or caregiving experience. Bilingual (English/Spanish or English/French) — a plus given the communities we serve. CPR and First Aid certification (or willingness to obtain).
TRAINING & ONBOARDING
All new Revora caregivers will complete orientation covering: Client rights and dignity standards Revora's emergency preparedness plan and procedures Mandatory abuse, neglect, and exploitation reporting — including APS and HHSC hotline numbers Proper documentation and service log completion Sharps disposal instruction requirements Scope of care — what caregivers may and may not do Written competency testing for any delegated clinical tasks prior to performing them
For CNAs:
Revora will maintain a training tracker documenting in-service hours and renewal deadlines to ensure Nurse Aide Registry compliance.
