Job Description

Overview Join our dedicated team as a Patient Care Assistant (PCA) Office, where you will play a vital role in coordinating and supporting patient care services within our healthcare facility. This position offers an exciting opportunity to work behind the scenes, ensuring seamless communication, accurate documentation, and efficient scheduling to enhance the quality of life for our patients. You will be the central hub for administrative support, patient record management, and care coordination, contributing directly to positive patient outcomes and team success. Responsibilities Manage patient records using Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems with accuracy and confidentiality in compliance with HIPAA regulations. Assist with scheduling patient appointments, follow-ups, and coordinating care plans in collaboration with nursing staff and healthcare providers. Support patient observation activities by documenting vital signs, medication administration, and assisting with Activities of Daily Living (ADLs) such as bathing, dressing, and mobility assistance. Maintain clear medical documentation including progress notes, medical records, and incident reports to ensure comprehensive patient histories. Facilitate communication between patients, families, and healthcare teams to ensure all care needs are understood and met efficiently. Assist with basic caregiving tasks such as meal preparation, light cleaning, laundry, and maintaining a clean and safe environment for residents or patients. Support specialized care needs including Alzheimer's care, dementia care, hospice support, catheter care, Hoyer lift operation, and memory care activities. Qualifications Prior experience working in assisted living facilities or nursing homes is highly preferred. Knowledge of medical terminology and familiarity with EMR/EHR systems is essential. Certification in CPR is required; additional certifications such as medication administration or catheter care are a plus. Experience working with individuals with developmental disabilities or those requiring dementia or memory care is advantageous. Strong understanding of patient observation techniques and senior care best practices. Ability to handle sensitive medical information in accordance with HIPAA standards. Excellent communication skills to effectively collaborate with healthcare teams and families. Demonstrated ability to perform caregiving duties including assisting with ADLs, meal prep, cleaning, laundry, and providing compassionate support.
