Job Description

Job description Serious Inquiries Only Direct Support Professionals will be responsible for the supervision of participants in a non-centered community engagement program. DSP is responsible for delivering services written in each person's plan of support. DSP is also responsible for daily documentation in participants' records. Must have at least one year of experience in providing direct support to people who have an intellectual or developmental disability diagnosis. Must know intellectual/ developmental diagnosis, mental health disorders, behavior management, and ability to implement and follow policies and procedures of DBHDS, HCBS, Office of Human Rights, DMAS, and Destin Pathways, LLC. The DSP must have the ability to engage in caring, training, and rehabilitation of people with the diagnosis of ID/DD. The ability to perform simple math, communicate both orally and written effectively, and maintain a professional working relationship with the people we serve and employees at Destin Pathways, LLC. DSP must also possess a valid driver's license. This position requires the successful completion of training/certification in CPR, First Aid, Behavior Management, and Medication Administration, and the successful completion of criminal and central registry background investigations. Provides transportation to and from activities; Assists in the implementation of individual plans for individuals and behavioral plan goals; Assists individuals in the pursuit of recreational, social, and community activities based on their preferences and individual plans; Assists and encourages individuals in the establishment of social networks; Promotes participation and involvement in community activities; Works all assigned schedules, including but not limited to: arriving and departing on time, requesting time off in advance, and notifying your supervisor of changes to your assigned work schedule; Ensures each individual's rights are protected; Treats individuals with dignity and respect. Document all program data, including but not limited to: individual plan data, behavior plan data, community integration logs, assessments, individual funds requests, mileage logs, maintenance requests forms, and supply acquisition forms; Documents individual's health (i.e. seizure charts, weight, bowel movements, fluid intake.); Ensures that all documentation is completed accurately and promptly; Ensures Medication Administration Record (MAR) entries are completed daily; Completes communication logbooks during the shift; Clocks in and out before and after every shift and submits Leave Request forms when applicable;
