Job Description

Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse settings. In this vital role, you will assist individuals with their daily living activities, monitor health conditions, and promote independence while ensuring safety and comfort. Your energetic approach and attention to detail will make a meaningful difference in the lives of those you serve. This position offers an opportunity to work closely with clients requiring long-term care, hospice support, or specialized memory care, contributing positively to their quality of life. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts or wheelchairs. Monitor and document patient observations, vital signs, and medical conditions accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications following prescribed protocols while maintaining compliance with HIPAA regulations to ensure confidentiality. Support patients with specialized needs such as Alzheimer's care, dementia care, or working with individuals with developmental disabilities. Perform household tasks including cleaning, laundry, meal preparation, and ensuring a safe home environment aligned with home & community care standards. Assist with medical procedures such as catheter care and providing hospice or palliative care when necessary. Use medical terminology confidently to document patient records and communicate effectively with healthcare teams. Experience Prior experience in home health or community care settings is highly preferred. Demonstrated ability to work with seniors or individuals with disabilities in assisted living or nursing home environments. Knowledge of medical documentation practices and familiarity with EMR/EHR systems. Certification in CPR and first aid is required; additional certifications such as medication administration or hospice care are advantageous. Experience handling medical equipment like Hoyer lifts or blood pressure monitors is desirable.
