  • Part-time, Full-time About Reliant Health VA, LLC Reliant Health VA, LLC is a licensed In Home Support Services (IHSS) provider serving individuals with Intellectual and Developmental Disabilities (IDD).

  • Provide personal care assistance including bathing, grooming, dressing, oral hygiene, and toileting
  • Assist with safe transfers, mobility support, and wheelchair navigation in the home and community
  • Implement goals and activities outlined in the Individual Support Plan (ISP)
  • Assist with meal planning and preparation in accordance with dietary needs and preferences
  • Transport individuals to medical appointments, community outings, and activities
  • Document all services accurately and on time using the Electronic Visit Verification (EVV) system
  • Report changes in health, behavior, or safety concerns to the Program Director promptly
  • Respond to incidents and emergencies per DBHDS regulations and agency procedures
  • Maintain confidentiality and uphold individual rights per
OHR 12VAC35-115
at all times
  • Participate in required trainings, team meetings, and supervision sessions
  • Model community integration, independence, and self-advocacy Must‑Haves 3+ year experience with IDD or related care.
Valid driver's license, reliable vehicle, and active auto insurance. Able to pass TB screening, background checks (Fieldprint), OIG/LEIE. CPR/First Aid (or willing to obtain). Medication Admin preferred.
