Job Description

Overview Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA), where your dedication will directly enhance the quality of life for our residents and clients. In this vital role, you will provide personalized, attentive care to individuals across various settings, including assisted living facilities, nursing homes, and home health environments. Responsibilities Assist residents and clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts. Monitor patient observations by tracking vital signs, assessing mental status, and reporting any changes or concerns promptly. Support individuals with memory care needs, including Alzheimer's and dementia care, ensuring a safe and comforting environment. Document medical information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining strict adherence to HIPAA privacy regulations. Administer medications and treatments following prescribed instructions under supervision or as authorized by healthcare professionals. Assist with medical procedures such as catheter care, wound care, and hospice support to ensure comfort and safety. Prepare meals in accordance with dietary guidelines, assist with feeding when necessary, and maintain cleanliness in patient areas including laundry and general cleaning tasks. Use medical terminology effectively to communicate with healthcare teams and document patient care activities precisely. Support patients during emergencies by performing CPR or other first aid measures when required. Collaborate with interdisciplinary teams to develop personalized care plans that meet each individual's unique needs. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated proficiency in patient observation, vital signs measurement, medication administration, and medical documentation. Experience working with individuals with developmental disabilities or those requiring specialized dementia or Alzheimer's care is advantageous. Familiarity with EMR/EHR systems to document patient information accurately and efficiently. Knowledge of medical terminology relevant to long-term care, hospice support, and home health services. Certification in CPR is required; additional certifications such as hospice care training or catheter care are a plus. Proven ability to handle sensitive situations with compassion while maintaining professionalism and confidentiality under HIPAA guidelines.
