Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by taking vital signs and observing for any changes in condition Support medication administration following care plans and HIPAA regulations to ensure privacy and safety Prepare nutritious meals and assist with feeding as needed, ensuring dietary restrictions are followed Operate equipment such as Hoyer lifts or wheelchairs to safely transfer residents or clients Maintain cleanliness of living spaces by performing light cleaning duties and ensuring a hygienic environment Document patient observations, care provided, and any incidents accurately in compliance with healthcare standards Collaborate with healthcare professionals to develop and implement personalized care plans tailored to individual needs Skills Proven experience in home health, assisted living, nursing homes, or group home environments Knowledge of patient observation techniques and ability to monitor vital signs effectively Certifications such as CPR, First Aid, and HIPAA compliance are highly valued Familiarity with dementia care, Alzheimer's care, memory care, and working with individuals with developmental disabilities or autism Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary Strong communication skills for working closely with clients, families, and healthcare teams Experience in medication administration and understanding of care planning processes Join us in providing compassionate support that empowers individuals to live healthier, happier lives.
