This vital role involves fostering independence, ensuring safety, and enhancing quality of life for clients across various care settings, including assisted living, home health, and long-term care facilities. The ideal candidate will be energetic, detail-oriented, and committed to delivering respectful, patient-centered care that promotes dignity and well-being. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor patients' health by taking vital signs and observing for changes in condition or behavior Administer medications accurately following care plans and HIPAA regulations to ensure patient safety and confidentiality Prepare nutritious meals tailored to dietary needs and assist with meal consumption as needed Maintain a clean and safe environment through regular cleaning duties and infection control practices Support clients with memory care needs, including Alzheimer's and dementia care, ensuring a secure environment Document patient observations, care provided, and any incidents meticulously in compliance with healthcare standards Skills Proven experience in group home settings, assisted living, or home & community care environments Knowledge of patient monitoring techniques, medication administration, and vital signs measurement Strong understanding of HIPAA regulations to protect patient privacy and confidentiality Ability to perform heavy lifting safely and operate assistive devices such as Hoyer lifts or wheelchairs Experience working with individuals with developmental disabilities, autism spectrum disorder, or dementia-related conditions Excellent communication skills for effective interaction with clients, families, and healthcare teams Certifications in CPR, First Aid, and basic nursing skills preferred; ongoing training encouraged to stay current in hospice care or memory support techniques Join us in making a meaningful difference by providing compassionate care that empowers individuals to live their best lives.
