$21./hr Position Summary The Direct Support Professional (DSP) - Community Coaching Support provides direct assistance and coaching to individuals with Intellectual and Developmental Disabilities (IDD) to help them build independence, strengthen daily living skills, and actively participate in community activities. The DSP supports individuals in achieving person-centered goals while promoting safety, dignity, inclusion, and self-determination. Essential Job Duties and Responsibilities The DSP Community Coach is responsible for providing support that may include, but is not limited to: Community Support & Skill Building Assist individuals in developing independent living and social skills in real-world community settings. Support individuals with accessing community resources such as libraries, gyms, recreation centers, parks, and local events. Coach individuals on appropriate social interactions, communication, and community etiquette. Assist individuals in building confidence and independence while participating in community activities. Goal Implementation Implement individualized support plans, person-centered plans, and outcome-based goals. Provide coaching to support vocational readiness, volunteer opportunities, and community engagement. Encourage individuals to make choices and advocate for their needs and preferences. Transportation & Community Navigation Safely transport individuals to and from scheduled community activities, appointments, and events. Assist individuals with learning routes, using public transportation when appropriate, and navigating community environments. Daily Living & Self-Care Support Support individuals with daily living skills such as budgeting, grocery shopping, meal planning, and personal hygiene routines when applicable. Provide assistance as needed while encouraging independence and skill development. Safety & Supervision Ensure individuals are safe at all times while in the community. Follow behavior support plans and provide redirection and de-escalation strategies as needed. Identify and report any concerns related to health, safety, abuse/neglect, or environmental hazards. Documentation & Communication Complete accurate daily documentation and service notes according to program and Medicaid/waiver requirements. Report incidents, concerns, or progress updates to the supervisor in a timely manner. Communicate professionally with families, guardians, and other team members as appropriate. Professional Conduct Maintain confidentiality and comply with HIPAA and agency policies. Maintain professional boundaries and provide respectful support at all times. Attend required staff meetings, trainings, and certifications. Qualifications Required Qualifications High school diploma or GED required. At least 1yr healthcare experience Valid driver's license CPR/First Aid certification (or willingness to obtain). Ability to pass background checks and required screenings. Ability to work independently and follow support plans accurately. Strong communication skills and ability to build positive rapport. Preferred Qualifications Experience working with individuals with IDD, autism, or behavioral support needs. Knowledge of person-centered practices and community integration services. Experience with documentation systems (electronic health records preferred).Contact:
