Full Job Description HOURS:
12 hour shifts (7-7) Full time: -Thursday, Friday and Every other weekend 7AM-7PM - Monday, Friday and Every other weekend 7AM-7PM Job Overview The Direct Support Professional (DSP) provides compassionate, person-centered support to individuals residing in the group home. DSPs ensure that all supports promote the individual's rights, dignity, and independence in accordance with the Home and Community-Based Services (HCBS) Settings Rule. DSPs are responsible for implementing the Individualized Service Plan (ISP), encouraging meaningful participation in daily activities, and promoting community integration consistent with each individual's preferences and goals. Responsibilities Person-Centered Support Follow each individual's Individual Support Plan (ISP) and required protocols. Support individuals in making choices about daily routines, goals, and activities . Promote self-advocacy, dignity, privacy, and independence . Health & Safety Conduct and document daily observations for assigned individuals. Immediately report safety concerns to the Nursing Director. Maintain and monitor group home supplies and safety equipment . Community Integration Help individuals participate in community activities such as recreation, volunteering, education, employment exploration, worship, shopping, and social groups. Encourage relationships with family, friends, and community members . Support community outings at a 1:3 staff-to-individual ratio or smaller when possible . Help individuals identify interests and explore community opportunities . Promote choice, independence, and participation in community activities. Document community activities , engagement levels, preferences, and feedback. Home Environment & Daily Living Assist with meals, hygiene, and household tasks while encouraging independence. Maintain a clean, safe, and home-like environment . Support individuals in personalizing and decorating their living spaces . Communication & Documentation Accurately document services, activities, and observations in our software. Participate in team meetings and documentation reviews . Maintain clear communication with supervisors, team members, and families about progress and needs.

$21.00 per hour
