Job Description

TriSource is seeking a dedicated and experienced Lead Direct Support Professional who is committed to providing exceptional care and support to individuals with developmental disabilities, and residents requiring specialized assistance. This leadership role involves supervising direct support staff, ensuring high-quality care, and fostering a safe, respectful environment. The ideal candidate will possess strong caregiving skills, extensive experience in a long-term care settings, and a passion for empowering individuals to achieve their personal goals.
Schedule:
Sunday-Thursday 3pm-11pm Pay Rate $20/HR Duties Supervise and mentor direct support staff, ensuring adherence to care plans and agency policies Coordinate daily activities, including assisting with Activities of Daily Living (ADLs), medication administration, and personal care Monitor residents' health by taking vital signs and recognizing signs of medical or behavioral issues Assist with mobility using equipment such as Hoyer lifts and support safe transfers Implement behavior management strategies tailored to individual needs, including dementia and Alzheimer's care Maintain accurate documentation using Electronic Medical Record (EMR) systems; record progress notes and incident reports Ensure a clean, safe environment by overseeing cleaning routines and safety protocols Collaborate with healthcare professionals, social workers, families, and team members to coordinate care plans Promote a respectful, inclusive atmosphere that encourages independence and dignity Qualifications Proven experience in assisted living, nursing home, or home & community care environments; long-term care experience preferred Certification in first aid and CPR required; Demonstrated ability to assist with ADLs such as bathing, dressing, grooming, and mobility support Familiarity with EMR systems for documentation purposes Strong leadership skills with the ability to supervise and motivate staff effectively Excellent communication skills for interacting with residents, families, healthcare providers, and team members Valid driver's license with a clean driving record for transportation duties Ability to work flexible hours including evenings or weekends as needed Join our team as a Lead Direct Support Professional to lead compassionate caregiving efforts that enhance the quality of life for those we serve.
