Job Description

Job Summary Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals requiring assistance with daily activities and health management. In this vital role, you will provide personalized support to promote independence, safety, and comfort within the home setting. Your expertise in patient observation, medication administration, and assisting with Activities of Daily Living (ADLs) will ensure clients receive high-quality, respectful care that enhances their well-being. This position offers an engaging opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care. Responsibilities Perform patient observation to monitor health status and report changes promptly using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Assist clients with Activities of Daily Living such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Administer medications accurately following medical terminology guidelines and HIPAA privacy regulations Support clients with specialized care needs including Alzheimer's care, dementia care, memory care, catheter care, and hospice care Take vital signs regularly—such as blood pressure, pulse, temperature—and document findings meticulously in medical records Prepare nutritious meals and assist with meal planning while maintaining cleanliness through light housekeeping tasks like laundry and cleaning Use medical documentation skills to record patient progress and communicate effectively with healthcare teams for coordinated home & community care Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred Demonstrated ability to work with individuals with developmental disabilities or disabilities requiring long-term care support Knowledge of medical terminology and proficiency in EMR/EHR systems for accurate recordkeeping Certification in CPR is required; additional certifications such as hospice or dementia care are advantageous Experience handling medical equipment including Hoyer lifts and managing catheter or medication administration is desirable Proven ability to provide compassionate patient care in a home or community setting while adhering to HIPAA standards Familiarity with working with elderly populations or individuals with complex health needs enhances your effectiveness in this role Join us to deliver exceptional home health services that empower clients to live safely and comfortably at home.
