This role offers a rewarding opportunity to make a meaningful difference in her life by providing tailored assistance that promotes independence, well-being, and community engagement. As an Independent Provider, you will work directly with Diana in her home or community settings, fostering positive relationships and empowering individuals to achieve their goals. Duties Assist Diana with activities of daily living (ADLs), including bathing, dressing, grooming, and meal preparation. Facilitate communication. Provide transportation and driving support to enable clients to attend appointments, activities, and community outings Collect data related to client progress, behavior patterns, and service delivery for documentation and reporting purposes Administer medications following prescribed protocols and ensure proper documentation of medication administration Collaborate with families, social workers, educators, and healthcare professionals to develop and implement personalized care plans Experience in home and community care environments demonstrating caregiving skills and adaptability Strong communication skills to effectively interact with clients, families, and team members Familiarity with assisting clients with ADLs.
