In this vital role, you will provide attentive, respectful, and professional assistance to clients in their homes or community settings. Your efforts will support their independence, comfort, and well-being while fostering a safe and nurturing environment. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those recovering from illness or managing chronic conditions. All roles are paid positions, emphasizing the value of your contribution and commitment. Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor and observe patients' health status by recording vital signs and noting any changes in condition Administer medications accurately following medical instructions and document all medication administration in medical records or electronic health record (EHR) systems Support clients with memory care needs, including Alzheimer's disease and dementia care strategies, ensuring safety and comfort at all times Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe living environment Document patient care activities thoroughly in compliance with HIPAA regulations and organizational policies to ensure confidentiality and accurate medical documentation Collaborate with healthcare professionals during home visits or hospice care to deliver comprehensive patient-centered services Skills Strong patient observation skills to detect changes in health status promptly Experience working in assisted living facilities, nursing homes, or home health environments Proficiency with EMR (Electronic Medical Records) and EHR systems for accurate documentation Knowledge of medical terminology relevant to caregiving tasks and patient care procedures Ability to perform CPR and handle emergency situations confidently Familiarity with specialized care such as catheter care, hospice support, dementia care, Alzheimer's care, and memory care techniques Skilled in assisting with ADLs including feeding, grooming, mobility assistance, and medication management Experience working with individuals with developmental disabilities or disabilities requiring long-term care support Competence in using medical equipment like Hoyer lifts and taking vital signs accurately Ability to work independently while maintaining effective communication within a caregiving team Compassionate attitude with excellent interpersonal skills to build trusting relationships with clients and their families Join us if you're passionate about providing exceptional home health care!
