Saturday & Sunday (7:00/9:00 AM - 9:00 PM)

OR Overnight Shift:

Friday & Saturday (9:00 PM - 9:00 AM) Work every other weekend or every weekend Option to pick up additional shifts About 4 Seasons AFH 4 Seasons Adult Family Home is a licensed Wisconsin AFH focused on providing person-centered care in a comfortable, home-like setting. We prioritize resident dignity, independence, and quality of life , while creating a positive and engaging workplace for our team. Position Summary We are seeking compassionate and dependable Personal Care Workers (PCWs) to join our Weekend team. This role involves providing hands-on care and support to residents in accordance with Wisconsin DHS 88 regulations and individualized service plans. Key Responsibilities (DHS 88-Aligned) Provide direct care and supervision to residents with disabilities or special needs Assist with Activities of Daily Living (ADLs) including bathing, dressing, grooming, toileting, mobility, and meal support Administer medications in accordance with

DHS 88.07

(3)(c) and training Monitor, document, and report changes in physical, mental, or emotional condition Follow Individual Service Plans (ISPs) and care plans Maintain accurate documentation and communication logs Ensure a safe, clean, and organized living environment Implement infection control and standard precautions Prepare meals and assist with dietary needs and preferences Promote resident independence and engagement in social/recreational activities Advocate for resident rights, dignity, and choice Qualifications (Wisconsin Caregiver Standards) Must be 18 years or older Eligible for Caregiver Background Check & Registry compliance Willingness to complete required DHS 88 training , including: Fire safety & emergency procedures First aid & choking response Medication administration Resident rights & confidentiality Previous caregiving, CBRF, or AFH experience preferred but not required Skills & Abilities Ability to administer medications safely and accurately Strong problem-solving and adaptability Effective communication and teamwork skills Proficiency in assisting with ADLs Ability to meet physical demands (lifting, standing, movement throughout shift) Understanding of the needs of individuals in assisted living or similar settings Why Work at 4 Seasons?
