Job Description

Nicolet Staffing is adding to the Team, we are seeking a Full-time Supportive Home Care Team Lead for out Ladysmith location! Manages the daily operations of supportive home care to include both clients and caregivers.
Responsibilities:
  • Build and maintain relationships with clients and caregivers.
  • Daily schedule management of clients and caregivers.
  • Review referrals from Managed Care Organizations and/or private clients, decision making based on capacity and caregiver availability.
  • Perform assessments and acceptance of potential clients to decipher appropriateness of placement..
  • Interview and onboarding of new caregivers.
  • Maintain client and caregiver files in accordance with company, local, state and federal policies.
  • Weekly verification of timesheets and EVV/Sandata.
  • Management of current operating system for caregivers and clients.
  • Maintaining and updating orientation materials and programming, as appropriate.
  • Shared responsibilities of after-hours phone.
  • Perform routine evaluations of client needs with close attention to client satisfaction.
  • Communicates with case manager or family members regarding client concerns or changes in client's physical and/or mental health along with ongoing client needs.
  • Audit caregiver job performance yearly or as needed.
  • Communicating and fostering a positive working relationship with client's care team.
  • Attending bi-annual client reviews with Managed Care Organizations.
  • Weekly reporting to Branch Manager.
Required Skills/Abilities:
  • Minimum of 2 years of previous caregiving or healthcare experience. .
  • Previous management experience preferred.
  • Ability to prioritize tasks, delegating when appropriate.
  • Within the scope of the job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment.
  • Proficient with Microsoft Office Suite or related software.
