Medical Social Worker - Home Health Tenderness Home Health LLC Shorewood, WI Job Details PRN $26.68 - $32.13 an hour 21 hours ago Qualifications CPR Certification Diagnostic evaluation Master's degree Home health Communication Sciences & Disorders Full Job Description We are seeking a licensed Speech Pathologist / Audiologist to provide skilled speech therapy services to patients in their homes under physician orders and the supervision of the Clinical Manager. The ideal candidate will assess, treat, and monitor patients with speech, language, swallowing, and hearing disorders while collaborating with the interdisciplinary care team. Responsibilities
  • Perform initial and ongoing speech therapy evaluations, including OASIS assessments.
  • Develop and implement individualized treatment plans in consultation with the physician.
  • Provide speech therapy services in the patient's home.
  • Monitor patient progress and revise treatment plans as needed.
  • Document evaluations, treatments, and progress notes within required timeframes.
  • Communicate patient status and treatment updates to physicians at least every 30 days or as changes occur.
  • Educate and counsel patients and families on therapy techniques and home programs.
  • Coordinate care with the interdisciplinary team.
  • Assist with discharge planning and complete discharge summaries.
  • Supervise home health staff when Speech Therapy is the only skilled service provided.
  • Participate in case conferences, staff meetings, and ongoing professional development. Qualifications
  • Master's or Doctoral Degree in Speech-Language Pathology.
  • Licensed (or eligible for licensure) in the State of Wisconsin.
  • Minimum of one (1) year of institutional or healthcare experience preferred.
  • ASHA membership preferred.
  • Current CPR certification (in-person; online-only not accepted).
  • Valid driver's license and reliable transportation.
  • Must pass a criminal background check.
  • Strong communication and documentation skills. Physical Requirements
  • Ability to travel to patient homes (may not be wheelchair accessible).
  • Ability to bend, stoop, reach, and lift/carry up to 30 pounds.
  • May assist with moving patients (up to 250 pounds with assistance).
  • Sufficient hearing, vision, and dexterity to perform patient assessments.
Pay:
$26.68 - $32.13 per hour
