Job Description

Licensed Practical Nurse (LPN) PHP of Alabama - Birmingham - Birmingham, AL Progressive Healthcare Providers is dedicated to providing the highest level of healthcare to our intellectually disabled individuals in the Birmingham and surrounding communities. PHP Birmingham currently has a Licensed Practical Nurse (LPN) position available. This is a community nursing position. This person will be responsible for the basic medical care including insulin dependent, enteral feedings, and medication monitoring to our clients in the community who are unable to care for themselves.
Duties and Accountabilities:
Provides general nursing care as required.
Documentation reflects nursing process:
assessment, intervention, and evaluation of interventions. Provides a safe, therapeutic environment that protects patients' rights through limit setting. Establishes contact with the client's physician, specialists, family members, social worker and program managers in order to assist in planning the client's care. Carries out and documents patient/family education and planning per P&P. Observes client behavior for changes in order to intervene therapeutically. Records changes and communicates changes to appropriate staff. Safely prepares, administers, and monitors and documents results of medication. Reviews medical record, checks physician orders, and records properly. Demonstrates awareness and follows program policies to include contraband and sharps, search policy, and understands the significance of enforcing policies to promote a safe and drug-free environment. Must have the ability to multi-task and communicate with Director on all aspects of Health Services issues Must be willing to take call at a minimum of 1 week every month. Rotating Shifts but will be daytime hours. Typically 7a-7p
Requirements Education/Training:
Graduation from an accredited school of nursing. LPN license in good standing with the Alabama Board of Nursing Current CPR certification
Experience/Requirements:
Prefer one year (minimum) experience in psychiatric patient care or LTC care Must pass background and drug screen. Must have no more than 3 moving violations in the past 3 years Must be able to provide proof of vehicle insurance in your name Upon Hire, must successfully pass 8 hour course to become Medication Administration Supervisor Certified Required experience: preferred : 1 year
