Medical terminology Full Job Description Job Summary Join our dedicated healthcare team as a full-time Licensed Practical Nurse (LPN) at our esteemed long-term care facility. In this vital role, you will provide compassionate, high-quality nursing care to residents, ensuring their comfort, safety, and well-being. Your expertise will support a multidisciplinary team in delivering comprehensive patient-centered care across various settings, including assisted living, memory care, and acute care environments. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with diverse healthcare needs. Duties Administer medications and treatments accurately, including injections, IV infusions, and catheter care, following established protocols and HIPAA regulations. Conduct thorough patient assessments by monitoring vital signs, performing physical examinations, and evaluating symptoms to inform appropriate care plans. Assist with complex procedures such as wound suturing, spinal taps, and respiratory support including ventilator management. Document patient information meticulously using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Cerner or Athenahealth to ensure compliance and continuity of care. Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans for residents with conditions such as dementia, Alzheimer's disease, infectious diseases, or post-acute recovery needs. Provide patient education on medication management, nutrition, pain control strategies, and health coaching tailored to senior and special needs populations. Participate in discharge planning, case management activities, and coordination of services for home & community care or post-acute settings. Qualifications Valid LPN license in good standing with relevant state licensing boards. Proven experience in long-term care facilities or nursing homes; experience with assisted living or memory care is highly desirable. Familiarity with EMR/EHR systems such as Cerner or Athenahealth; proficiency in
ICD-9/ICD-10
coding for accurate documentation. Knowledge of advanced nursing procedures including IV insertion, phlebotomy, sterile technique, and tube feeding management. Strong understanding of physiology, anatomy, pharmacology terminology, and diagnostic evaluation processes. Experience working with individuals with disabilities or developmental delays; familiarity with dementia and Alzheimer's care preferred. Ability to handle emergency situations effectively with skills in airway management and acute pain management; ICU or trauma center experience is a plus. Excellent communication skills for triage assessments and working collaboratively within multidisciplinary teams.
