Field LPN for Hospice Saad Healthcare - 4.1 Mobile, AL Job Details Full-time 1 hour ago Qualifications LPN Full Job Description About the Role Saad Hospice is seeking a compassionate, self-motivated Full-Time Field Hospice LPN to join our dedicated care team. In this role, you will provide critical comfort, support, and specialized nursing care to terminally ill patients and their families in their home environments. If you have a deep passion for hospice care and want to make a meaningful difference every single day, we want to hear from you.
Schedule:
Monday - Friday, 10:00 AM - 7:00 PM (Includes a rotating on-call schedule).
Employment Type:
Full-Time Requirements & Qualifications Active and unencumbered LPN License. Reliable personal transportation with valid car insurance. Ability to pass a comprehensive background check and drug screen. Strong self-motivation, excellent time management, and a heart for hospice care. Duties & Responsibilities Direct Patient Care & Assessment Assess patients' clinical conditions, symptoms, and nursing needs; implement appropriate interventions under the direction of the physician and senior hospice leadership. Prioritize aggressive pain and symptom management to ensure maximum patient comfort. Conduct PRN (as-needed) visits for field hospice patients as required. Provide physical, mental, and emotional support to both patients and their families. Family Education & Support Educate patients and caregivers on pain/symptom management, medications, dietary needs, disease progression, and what to expect. Actively support the patient and family through the dying process with dignity and compassion. Advocate for the patient, ensuring their choices, desires, and rights are consistently honored. Collaboration & Documentation Document all observations and care accurately, concisely, and timely, adhering strictly to Saad Hospice Documentation Standards and regulatory guidelines. Participate actively as a member of the Interdisciplinary Team (IDT) to identify patient needs and collaborate on care plans. Refer patients/families to other hospice team specialists (social work, chaplain, etc.) as specific needs arise. Participate in the staff on-call rotation as required.
