Job Description
Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and recovery. Administrative Management (30%) Through consultation with the appropriate agency personnel, develops overall healthcare management goals for Residential Services and specific plans to meet goals. Develops operational systems for managing client healthcare in individual Residential Services programs. Receives requests from staff for needed healthcare supplies; verifies need; approves, and obtains through established procedures. Monitors the medication management and dispensation implementation plans in the Residential Services Department making recommendations for improvement as needed. Check all medication logs for Residential Services daily. Consult with the Team Leaders regarding any refills, new prescriptions and doctor appointments. Pickup/drop off all prescriptions to the pharmacy. Ensure that Team Leaders have a working knowledge of all medication placed in their care. Establishes and maintains files and records on youth and healthcare programming activities. Completes weekly and monthly reports for submission to the Deputy Director or his/her designee. Provides for periodic staff training in first aid, CPR, and medication administration. Reviews daily paperwork of fellow staff for accuracy; verifies admissions and discharges. Reviews program statistics, reporting those which are unusual to the Deputy Director. Attends and participates in meetings of administrative staff and other meetings as required. Conducts refresher classes as needed. D. Public Relations & Related Duties (5%) Serves as liaison with Juvenile Court, DHR, Social Workers, Probation Officers, and/or other outside agencies; serves as catalyst to resolve any problems pertaining to child's healthcare goals which may arise. Provides necessary coordination between social service agency and youth in order to obtain needed health care services for youth. Serves with other administrative staff in on-call capacity to handle emergency situations. Responds to families of children/youth regarding problems, complaints, or specific requests; may meet in person, write letters, and communicate by phone. May attend court hearings and provide court report as indicated. May refer residents/families to appropriate agencies/organizations as needed. May attend and participate in special activities and functions sponsored by various residences. Attends and participates in educational/training programs as required. Maintain First Aid Kits for Residential Services programs in consultation with Team Leaders. Performs other duties as may be assigned.
DHR. DYS
licensing standards and LCYDC policies and procedures. Completes quarterly statistical DYS reports in Detention, STARS, and BEAMS. Completes sexually aggressive behavior and risk documentation. B. Direct Client Care (50%) Conduct health intake screening on residents. Provide sick call services to Residential Services programs. Provide after hours on-call services as required. Conduct intake evaluations for TFC clients when requested by a TFC Coordinator. Conduct abstinence classes with the residents. Meets with new admissions to ensure they have received healthcare orientation and to assess whether further healthcare referral is indicated. Ensures residents receive basic healthcare services including proper healthcare screening, any physical and/or mental health medication is in programs and being dispensed as prescribed. Ensures all medication safety rules and precautions are followed. Establishes a communication link with each youth. Arranges to meet with youth on a scheduled regular basis and as requested by staff; encourages youth to talk openly about issues/problems specific to their healthcare needs; Conducts group sessions to allow youth to discuss healthcare concerns and provide accurate healthcare information. Helps youth to develop healthy lifestyle habits and plans. Makes necessary referrals and follows up with staff on specific issues as appropriate. Participates in all programs on a random basis; intervenes as appropriate and assists staff in modifying healthcare implementation programs as needed. Administers TB Skin Tests. C. Staff Training (15%) Reviews healthcare training needs for all staff. Develops an annual training plan to meet staff training needs. Schedules all training sessions in conjunction with Training Coordinator. Organizes, maintains, and stores all training materials. Conducts applicable healthcare medication management and dispensation training classes. Provides documentation and maintains records of training goals which were met by staff.Conducts refresher classes as needed. D.
