Job Description

Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to patients across various settings, including long-term care, assisted living, hospitals, and community health environments. This role offers an opportunity to make a meaningful difference in patient outcomes through direct care, patient assessment, and collaboration with multidisciplinary teams. The ideal candidate will possess strong clinical skills, a commitment to patient-centered care, and experience working with diverse populations, including individuals with developmental disabilities, dementia, and acute or chronic health conditions. Duties Provide comprehensive nursing care by performing physical examinations, vital sign assessments, and patient evaluations to determine health status and identify immediate needs. Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA regulations and medical documentation standards. Manage IV infusions, catheter care, tube feeding, and ventilator support as required for specialized patient needs. Conduct diagnostic evaluations such as specimen collection, processing, and basic laboratory procedures including phlebotomy and ICD coding for accurate record-keeping. Assist with wound suturing, sterile processing, and infection control practices to ensure safety and compliance. Support discharge planning and coordinate with case management teams to facilitate smooth transitions across care settings. Utilize EMR (Electronic Medical Records) systems like Epic or Cerner for documentation of patient care activities. Perform patient assessments focusing on physiology and anatomy knowledge to inform treatment plans in diverse environments such as emergency rooms, ICU (Intensive Care Unit), or outpatient clinics. Engage in health coaching and education for patients with chronic conditions like Alzheimer's disease or dementia to promote optimal memory care and aging-in-place strategies. Qualifications Current licensure as a Licensed Practical Nurse (LPN) in accordance with state regulations. Proven experience in critical care settings such as ICU or trauma centers is highly desirable. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth. Knowledge of advanced nursing procedures including IV insertion, medication administration, airway management, and pain management techniques. Familiarity with
ICD-9/ICD-10
coding standards for accurate medical documentation. Experience working with individuals with developmental disabilities or in specialized areas such as hospice care, behavioral health, or pediatric nursing is preferred. Strong understanding of infection control protocols, aseptic technique, and sterile processing procedures. Excellent communication skills to collaborate effectively within multidisciplinary teams while maintaining professionalism and compassion at all times. Ability to adapt quickly in fast-paced environments such as emergency medicine or urgent care facilities while ensuring patient safety remains paramount.
