This is a full time position with benefits such as vacation, health insurance, etc.

Job Responsibilities:

Conduct admissions for LTC and Skilled nursing patients Prep charts prior to patient arrival, enter orders, and conduct admission assessment. Provide information, screen, and teach patients and their families. Coordinate with other health care professionals to plan and improve the care of the patients. Evaluate the condition and needs of the patients and ensure they receive the appropriate services.
