Job Description

Overview Join our team as a Licensed Practical Nurse (LPN) in a busy clinic setting, where your expertise will directly impact patient care and well-being. This role offers an exciting opportunity to work in a fast-paced environment, providing essential nursing services across diverse patient populations. You will be instrumental in delivering high-quality, compassionate care while collaborating with multidisciplinary teams to ensure optimal health outcomes. Your skills will help create a welcoming atmosphere where patients feel valued and cared for every step of their health journey. Duties Perform comprehensive patient assessments, including vital signs and health history documentation using Electronic Health Record (EHR) systems such as Practice Fusion. Assist with medical procedures including IV infusions and medication administration following sterile technique and aseptic procedures. Manage patient triage by evaluating symptoms, prioritizing urgent cases, and coordinating appropriate care pathways in urgent care or outpatient settings. Educate patients and caregivers on health coaching topics including nutrition, medication adherence, pain management, and post-discharge care plans. Maintain meticulous medical records documenting all patient interactions, assessments, interventions, and discharge instructions in accordance with healthcare standards and with competent literacy. Communicate effectively and coherently with office staff, pharmacies, and outside healthcare offices. Process refill requests and obtaining prior authorizations as needed per insurance plans. Answer and return calls for nursing line Skills Proven experience working with Electronic Health Record (EHR) systems like Epic, Athenahealth, or eClinicalWorks; proficiency in
ICD-9/10
coding is highly desirable. Strong background in nursing; behavioral health experience is preferred. Skilled in performing advanced nursing duties such as IV and medication administration protocols. Ability to work effectively with diverse populations including children (pediatrics), individuals with disabilities or developmental delays (including behavioral health), and seniors requiring dementia or Alzheimer's care. Excellent patient assessment skills combined with compassionate communication to support patient-centered care in outpatient setting. $19.00 - $22.00 per hour
