LPN- Part time and PRN

Our Care Department currently has openings for LPNs or RNs. Our Weekly schedule is as follows: Saturday and Sunday 7pm
• 7am Saturday and Sunday 7am
• 7pm Monday
• Friday 7am
• 3pm Monday
• Friday 3pm
• 11pm Monday
• Friday 11pm
• 7am PRN (any shift
• at least once every 30 days) Summary of Duties Observe, assess (within scope of practice), and reports Patient conditions/changes to family and provider(s).

Provides necessary documentation in the EMR of all information relevant to patient assessment, evaluation, change of condition, in accordance to nursing practices. Assist in room changes and admissions. Document appropriate happenings in all designated areas (nursing notes, incident reports, care plans, 24-hour report, etc.). Ensure the health, comfort, and safety of patients by assisting with bathing, feeding, and dressing. Consults with Director of Care and/or Administrator regarding changes in Patient's condition. Ensures that each Patient's attending physician and family or responsible party is promptly notified of any significant change in the Patient's health condition. Carry out the service plan as directed and administer medications and treatments as prescribed. Responsible for assuring resident safety, including the delivery of appropriate clinical care by the CNA staff. Qualifications Must be a graduate of an approved school for practical or vocational nursing. Must be currently licensed by the State Board of Nursing or pending licensed practical nurse licensure. Experience in rehabilitative or geriatric nursing, preferred. Must maintain current BLS/CPR certification, preferred.
