Position Overview:
Navitas Healthcare, LLC is seeking a Licensed Practical Nurse
  • Long-Term Care for an exciting Local job in Phoenix, AZ.
Responsibilities Administer medications and treatments as prescribed and within the scope of LPN practice. Perform nursing procedures including wound care, catheter care, tracheostomy care, and enteral feeding care. Monitor residents and evaluate the effectiveness of medications and treatments. Observe, document, and report changes in resident condition to the healthcare team. Participate in shift reports and communicate resident care needs effectively. Complete admission assessments for newly admitted residents. Assist with MDS assessments, quarterly reviews, and care plan development. Maintain accurate clinical documentation, treatment records, and resident progress notes. Communicate effectively with residents, families, physicians, and interdisciplinary team members. Ensure adequate supplies and medications are available for resident care. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Follow all federal, state, facility, and nursing practice regulations. Required Qualifications Active Arizona Licensed Practical Nurse License. Current and unrestricted nursing license in good standing. Knowledge of geriatric and long-term care nursing practices. Ability to assess resident conditions and respond appropriately to medical concerns and emergencies. Strong clinical documentation and medication administration skills. Excellent verbal and written communication skills. Ability to work effectively within an interdisciplinary healthcare team. Ability to maintain professionalism and confidentiality in all resident interactions. Preferred Qualifications Long-Term Care experience. Skilled Nursing Facility experience.
