Job Description

Header Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, skilled nursing services across diverse settings, including long-term care, assisted living, hospitals, and community health environments. Your dedication will help improve health outcomes through attentive patient assessment, medication administration, and personalized care plans. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic, Cerner, or Athenahealth. Administer medications and injections accurately following HIPAA guidelines and established protocols, including IV infusion and medication management. Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing to ensure optimal patient outcomes. Monitor patients' conditions closely in acute care settings like ICU or emergency rooms, managing airway management, ventilator support, and acute pain management. Conduct triage assessments for urgent or emergent cases in outpatient or urgent care environments to prioritize patient needs effectively. Collaborate with multidisciplinary teams for discharge planning, case management, and post-acute care coordination to facilitate smooth transitions between care settings. Educate patients and their families on health coaching topics such as dementia care, Alzheimer's care, nutrition, family planning, and chronic disease management. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, hospice care, or outpatient clinics. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; familiarity with
ICD-9/ICD-10
coding is a plus. Strong knowledge of physiology, anatomy, medical terminology, infection control practices, and aseptic techniques. Experience with specialized areas such as critical care (ICU), emergency medicine, geriatrics—including memory care—and pediatric or neonatal settings is highly desirable. Ability to perform venipuncture, phlebotomy, catheter care, specimen collection/processing and manage complex patient assessments confidently. Excellent communication skills to effectively collaborate with healthcare teams and educate patients on treatment plans.
