Position Overview:
"Navitas Healthcare, LLC" is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide direct nursing care within a long-term care environment. The ideal candidate will support resident health, safety, and quality of life while collaborating with an interdisciplinary healthcare team to deliver exceptional patient-centered care.
Primary Responsibilities:
Administer medications and treatments as prescribed Perform nursing procedures including wound care, catheter care, tracheostomy care, and enteral feeding care Monitor residents and evaluate treatment effectiveness Observe, document, and report changes in resident condition Participate in shift reports and resident care planning Complete admission assessments for new residents Assist with care plans, assessments, and resident reviews Maintain accurate clinical documentation and treatment records Communicate effectively with residents, families, physicians, and healthcare team members Ensure availability of necessary supplies and medications Participate in quality improvement initiatives Follow all nursing practice standards, facility policies, and regulatory requirements Attend required staff meetings and educational programs
Required Qualifications:
Active Arizona Licensed Practical Nurse (LPN) License Current unrestricted nursing license in good standing Knowledge of geriatric and long-term care nursing practices Strong medication administration and clinical documentation skills Ability to assess resident conditions and respond appropriately Excellent communication and interpersonal skills Ability to work effectively within a multidisciplinary healthcare team Professionalism and commitment to resident-centered care
Preferred Qualifications:
Previous Long-Term Care experience Experience working with geriatric populations Strong clinical assessment and patient care skills
