Hours Per Week Position Overview:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide exceptional care in a long-term care/skilled nursing environment. This is an excellent opportunity for an experienced nurse who is passionate about resident-centered care and thrives in a collaborative healthcare setting. Responsibilities Administer medications and treatments as prescribed and within scope of practice Perform nursing procedures including wound care, catheter care, tracheostomy care, gastrostomy/jejunostomy care, and other assigned treatments Monitor residents' conditions and report responses to medications and treatments Complete admission assessments for newly admitted residents Assist with MDS assessments, quarterly reviews, and care plan development Maintain accurate and timely clinical documentation Participate in Quality Assurance and Performance Improvement (QAPI) initiatives Communicate effectively with residents, families, physicians, and interdisciplinary team members Provide detailed shift reports and ensure continuity of care Attend staff meetings and ongoing training programs Qualifications Active Licensed Practical Nurse (LPN) license in good standing Experience in Long-Term Care (LTC), Skilled Nursing Facility (SNF), Rehabilitation, or Geriatric Nursing preferred Knowledge of medication administration and nursing best practices Experience with MDS assessments and care planning preferred Strong assessment, critical thinking, and communication skills Ability to respond effectively to medical emergencies and changing resident conditions For more details reach at or Call / Text at 516-862-1203 .
