Our mission is to enhance the quality of life for individuals living with serious illnesses by delivering comprehensive care that focuses on comfort, symptom management, and emotional well-being. We are seeking caring and dedicated Licensed Practical Nurses to join our team and make a meaningful difference in the lives of patients and their families. As a Licensed Practical Nurse at AvoCare , you will provide skilled nursing care and ongoing support to patients living with serious illnesses in the comfort of their homes and other care settings. You will assist with patient assessments, monitor symptoms, administer medications as directed, educate patients and families, and collaborate closely with Registered Nurses, physicians, and the interdisciplinary care team to ensure high-quality, patient-centered care. This part-time position offers flexibility in scheduling and coverage areas, making it an excellent opportunity for compassionate nurses who are committed to improving comfort, promoting dignity, and enhancing quality of life for their patients.
Responsibilities :
Coordinate and provide home visits for palliative care patients who have a chronic or serious illness and need symptom management and/or education Document in EMR- assessment, patient status, and outcome, goals of care needs, after each visit Ensure regulatory compliance with documentation requirements Adhere to HIPPA at all times Identify patient needs and initiate appropriate goals updating as needed Communicate and coordinate on a consistent basis with members of team, other healthcare providers involved in pt care and Insurance Case Managers Act as a liaison and advocate for PC patients admitted to the hospital Participate and assist in any quality improvement or other studies involving palliative care Ensure the plan of care for assigned patients is adhered to Attend interdisciplinary team meetings and participate by providing patient updates Participate in regular scheduled staff meetings and inservices Additional duties as agreed upon to meet agency need
Job Qualifications:
RN or LPN with minimum of 3 years full-time equivalent experience Past nursing experience should be in a transferable or applicable clinical venue Currently licensed as a Registered or Practical Nurse to practice in the State of Arizona Demonstrates clinical competency; ability to collaborate with others; ability to initiate and plan own work activities; effective time management skills and professionalism Possess and maintain good physical stamina and mental health
Tactical Deliverables:
Notifies patient prior to visit and communicates further visits scheduled. Drives responsibly and safely to and from client's place of residence. Schedules priorities and maintains productivity utilizing time management. Provides appropriate and thorough service to patients/clients, ensuring proper completion of services in an accurate and timely manner and in accordance with agency policy. Always demonstrates respectable customer service and professionalism. Determines discipline frequencies according to physician order and/or assessed needs. Provide teaching to patient/client and answer health-related questions and document patient/client/caregiver understanding of education. Document patient's/client's care using appropriate agency forms and submit to agency within appropriate timeline. Maintain continuity of care and document communications with others within the health care team. Demonstrate work that meets quality standards. Assist in periodic auditing of paperwork of self/peers. Adheres to Agency Code of Conduct, Standards of Practice, and Patients/Patient Rights Demonstrates professionalism in appearance, adhering to agency dress code. Accessible transportation to and from patients visits. Ability to lift 30 pounds, move intermittently throughout the workday, inclusive of sitting, standing and bending, pushing, pulling or moving objects and patients/clients.
Education and Experience:
Currently licensed as a Practical Nurse to practice in the State of Arizona (or) Current multi-state license from another compact state utilizing their valid compact state license for up to 30 days until an Arizona permanent license is issued.
