LICENSED PRACTICAL NURSE
(LPN)

Hour Schedule:
Full-Time 12-Hour Shifts 6:00 AM - 6:30 PM 36 Hours One Week / 40 Hours the Following Week (Alternating Schedule)
POSITION OVERVIEW
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide quality nursing care in a long-term care and geriatric setting. The LPN will administer medications, perform treatments, monitor resident conditions, and collaborate with interdisciplinary team members to ensure exceptional resident care.
KEY RESPONSIBILITIES
Administer medications and treatments as prescribed by physicians and within the scope of nursing licensure. Perform treatments including wound care, catheter care, tracheostomy care, gastrostomy and jejunostomy care, postural drainage, and other clinical procedures. Monitor resident responses to medications and treatments and report findings appropriately. Observe, assess, and document changes in resident conditions. Ensure adequate supplies, medications, and treatment materials are available. Participate in shift report exchanges and communicate resident care needs effectively. Collaborate with physicians, residents, family members, and interdisciplinary team members to coordinate care. Complete initial nursing assessments for newly admitted residents. Assist with Minimum Data Set (MDS) assessments, quarterly reviews, and individualized care plans. Maintain accurate and timely clinical and administrative documentation. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Attend and actively participate in staff meetings and training sessions. Follow all facility policies, nursing regulations, and healthcare compliance standards.
QUALIFICATIONS
Active Licensed Practical Nurse (LPN) license in good standing. Previous experience in long-term care, skilled nursing, rehabilitation, or geriatric nursing preferred. Knowledge of geriatric nursing principles and age-related healthcare needs. Experience administering medications and performing clinical treatments. Familiarity with MDS assessments and care planning processes preferred. Strong understanding of state and federal nursing regulations.
KNOWLEDGE & SKILLS
Gerontological and geriatric nursing theory and practice. Medication administration and pharmacology principles. Treatment planning and resident care procedures. Clinical assessment and observation skills. Documentation and medical record management. Quality assurance and performance improvement processes. Strong oral and written communication skills. Excellent interpersonal and customer service skills. Ability to work effectively within a team nursing environment.
REQUIRED ABILITIES
Assess, prioritize, and respond appropriately to resident needs and medical emergencies. Make sound clinical decisions and follow up on resident health concerns. Maintain professional communication with residents, families, physicians, and staff. Establish and maintain positive working relationships. Adhere to nursing regulations, policies, and procedures. Work independently while managing multiple responsibilities.
