Full-Time, Night Shift Hours:
6:30 PM - 6:00 AM (12-hour shifts, approximately 40 hours per week) Position Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide quality nursing care to residents in a long-term care setting. The LPN will administer medications and treatments, monitor resident conditions, maintain accurate documentation, and collaborate with interdisciplinary teams to ensure exceptional resident care. Key Responsibilities Administer medications and treatments as prescribed by physicians and within the scope of nursing licensure. Perform a variety of nursing treatments including wound care, tracheostomy care, gastrostomy/jejunostomy care, catheter care, postural drainage, and other assigned procedures. Observe, assess, and report resident responses to medications and treatments. Monitor resident conditions and communicate changes to appropriate healthcare providers. Ensure adequate supplies and medications are available for resident care. Participate in shift-to-shift reporting and effectively communicate resident status updates. Communicate professionally with residents, family members, physicians, and interdisciplinary team members. Complete admission nursing assessments for newly admitted residents. Assist with Minimum Data Set (MDS) assessments, quarterly reviews, and care plan development. Maintain accurate clinical and administrative documentation regarding resident diagnoses, treatments, observations, and progress. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Attend and participate in staff meetings and required training sessions. Respond appropriately to medical emergencies and resident health concerns.
Required Knowledge & Skills Knowledge of:
Geriatric and gerontological nursing practices Effects of the aging process Team nursing principles and procedures Resident treatment planning and implementation Pharmacology and medication administration Federal, state, and agency regulations governing nursing practice Medical supply and medication control procedures Skills in: Resident assessment and observation Clinical documentation and reporting Patient care planning and implementation Effective verbal and written communication Professional interpersonal interactions Critical thinking and problem-solving Ability to: Practice within nursing regulations and standards Assess and respond to medical emergencies Make sound independent clinical decisions Maintain professional relationships with residents, families, and staff Respond to medical inquiries in a professional and timely manner Minimum Qualifications Current Licensed Practical Nurse (LPN) license in good standing CPR/BLS Certification Level 1 Fingerprint Clearance Card TB Skin Test documentation Hepatitis B vaccination documentation or signed declination
