Job Description

Job Overview We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) for a part-time position to provide exceptional geriatric care management across various healthcare settings. This role offers an exciting opportunity to work in a dynamic environment, delivering vital nursing services while supporting patient well-being. The ideal candidate will bring enthusiasm, strong clinical skills, and a commitment to improving health outcomes through attentive and personalized care. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation. Administer medications and other treatments following proper techniques and safety protocols. Document all patient interactions accurately within electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth, ensuring compliance with HIPAA regulations. Collaborate with multidisciplinary teams on discharge planning, case management, and long-term care strategies, including memory care and hospice services. Provide triage and urgent care services by evaluating patient needs promptly and directing them to appropriate levels of care in the emergency room or outpatient settings. Requirements Valid RN or LPN license in good standing with the state licensing board. Proven experience in diverse healthcare environments such as hospitals, nursing homes, assisted living facilities, or outpatient clinics. Familiarity with EMR/EHR systems including Epic, Cerner, or eClinicalWorks. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques. Experience with specialized procedures such as ostomy care or wound management preferred. Ability to perform physical examinations and vital sign assessments accurately while demonstrating excellent patient care skills.
