Non-Exempt Position Summary Under the supervision of the Registered Nurse, the Licensed Practical Nurse (LPN) provides direct patient care to assigned patients. In this role, you will maintain a safe, clean, and efficient working and treatment environment in accordance with facility policies and procedures while communicating effectively with the treatment team to ensure the highest quality of care. Key Responsibilities (Essential Functions) Direct Patient Care & Behavioral Intervention Carry out all aspects of patient care and assignments as directed by the Charge Nurse. Perform ongoing observation of patients and the milieu to ensure active treatment participation. Report changes in patient condition or behavior (including agitation, high-risk behaviors, vital signs, and eating/sleeping/toileting patterns) to the Charge Nurse/RN. Intervene appropriately during behavioral crisis situations (e.g., suicidal or threatening behavior, elopement) using approved interventions. Initiate and participate in emergency medical care when indicated. Provide support and direct assistance to patients with activities of daily living (meals, grooming, ambulation, and toileting). Facilitate educational and activity groups using approved materials, and recommend alternative activities based on patient functioning levels. Shift Support & Safety Operations Review assignment sheets, bed boards, and facility communication tools (logs, bulletin boards) at the start of each shift. Review practitioner orders, MARs, laboratory results, test reports, and transferred records daily. Perform environmental safety rounds, complete assigned check sheets, mitigate risks, and report findings. Monitor the patient environment to ensure all safety, sanitary, regulatory, and quality standards are met or exceeded. Remain on-duty until subsequent shift nursing coverage arrives on the unit and a verbal report is completed. Ensure proper inventory and storage of patient valuables upon admission and discharge. Medication Administration Ensure a First Dose Review is completed prior to administering any new medication. Provide comprehensive medication education to patients and address any related questions or concerns. Document medication variances and adverse drug reactions promptly; notify the patient, practitioner, and facility leadership per policy. Control secure access to medication rooms and dispensing machines, ensuring spaces remain clean and free of personal belongings. Collaboration & Documentation Interact with patients, family members, external entities, and internal staff professionally and respectfully to preserve patient dignity. Complete timely, legible, thorough, and accurately formatted documentation for medical records, including patient observations, MARs, education logs, and incident reports. Position Requirements & Qualifications Graduate of an accredited LPN program. Current state licensure as a Practical Nurse. CPI and CPR Certification per facility policy. Preferred:
Two (2) years of experience as an LPN, with at least one (1) year of acute psychiatric care experience. Core Competencies:

Inpatient behavioral health facility. Exposure to irregular/prolonged hours, blood-borne pathogens, and potential patient agitation or aggressive behaviors. Physical Activity:
