Requirements:
Active LPN/LVN license 2+ years of clinical experience (Home Health/Hospice preferred) Homecare Homebase experience CPR certification Valid driver's license, reliable transportation, and auto insurance
Responsibilities:
Provide skilled home health nursing under RN supervision Patient assessments, medication/care, documentation, and education Coordinate care with the RN, physician, and interdisciplinary team Assist with hospice care as needed Possible on-call rotation Interested?
