Job Description:
Swan Home Health Care has proudly served California communities . We are currently seeking a skilled and compassionate Licensed Vocational Nurse (LVN) with wound care experience to join our home health team. This is a flexible opportunity to work directly with patients in the comfort of their homes, providing high-quality wound care and contributing to positive health outcomes.
Responsibilities:
Perform wound care, dressing changes, and wound assessments Monitor and document healing progress in accordance with care plans Administer medications and treatments as prescribed Educate patients and families on wound care and infection prevention Collaborate with RNs, physicians, and interdisciplinary care team Complete timely and accurate clinical documentation using
EMR Qualifications:
Current LVN license in the State of California Minimum 1 year of wound care experience (home health preferred) Wound care certification (WCC or similar) preferred but not required Reliable transportation and valid driver's license Strong interpersonal and time management skills Must be able to travel for field visits
